An Employer Recommendation form recommends the individual to be considered for the NCEA Certification Program.
The employer believes that they have an understanding of the Scientific Concepts and Services outlined within the Candidate Information Bulletin(CIB).
ALL NCEA CERTIFICATION CANDIDATES THEN MUST MEET THE ADDITIONAL REQUIREMENTS:
2. Current CPR/AED Certification. (notarized copy)
3. Current First Aid Certification or equivalent First Responder. (notarized copy)
Do you have a current CPR/AED and First Aid Certificate? If not, check out classes with the American Red Cross or American Heart Association.

You may also check with your local first aid or fire department. Some of these programs may be offered as a community service in your area.
4. Current NCEA Individual Membership Certificate. (notarized copy)
Are you a current NCEA member or has your membership expired? Membership certificate is required to prove membership status.

5. Proof of Insurance Certificate.* (notarized copy)
*Provide CANDIDATE AFFIDAVIT OF PROFESSIONAL INSURANCE, if you are currently not practicing skin care and do not have current professional liability insurance.
Check your policy for the endorsement page or contact your risk manager of your professional liability company.
See NCEA Training Manual, Section 5, Risk Management for an extensive review of professional liability insurance.
If you are currently a non-practicing skin care professional (ie: academic, other professional, etc.) you may not be carrying professional liability insurance. Complete the Affidavit of Professional Insurance form. (supply notarized copy).
6. Two (2) passport photos.
Size 2" x 2" showing head & shoulders. No dark glasses. Print your full name on back of both photos.
7. AFFIDAVIT OF KNOWLEDGE REVIEW COMPLETION. (notarized)
NCEA Certification Candidate must complete all of the Knowledge Reviews in the NCEA Training Manual and verify each answer is correct.
IMPORTANT: You must tear the completed, signed & dated Knowledge Reviews directly from your NCEA Training Manual and ATTACH to this Affidavit of Knowledge Review Completion.
NOTE: YOU MUST SUBMIT THE ORIGINAL KNOWLEDGE REVIEWS TORN FROM YOUR NCEA TRAINING MANUAL ALONG WITH YOUR AFFIDAVIT OF KNOWLEDGE REVIEW COMPLETION FORM.
NO PHOTOCOPIES OR FASCIMILES WILL BE ACCEPTED. THE PERFORATED EDGES MUST BE VISIBLE AS VERIFICATION OF ORIGINAL DOCUMENTATION FROM TRAINING MANUAL.
8. Check made payable to NCEA Certified in the amount of $175.00. (US Funds)
Compile the above documents with the required affidavits using your Candidate Application as a check list. Have your affidavits and documents copied and notarized (except Knowledge Reviews) . Assemble your Candidate Application and supporting documents and send to NCEA for records verification. Enclose a check made payable to NCEA Certified in the amount of $175.00. This fee is non-refundable.
Upon verification of your Candidate Application you will receive your Authorization to Test (ATT) letter to the email address on Candidate Application.
BE ACCURATE, BE NEAT. We cannot be responsible for errors due to illegible handwriting.
DO NOT SEND ORIGINALS. NCEA or its agents will not be responsible for returning any documentation required for Records Verification.
